Primary Care Assessment Tool

Assessment of Primary Care Resources and Supports for Chronic Disease Self Management (PCRS)

The PCRS tool was developed by the Diabetes Initiative to further define the elements of self management support in primary care. Intended for use with multi-disciplinary patient care teams, the tool is designed to 1) function as a self assessment, feedback, and quality improvement tool; 2) characterize optimal performance of teams and systems; 3) promote discussion among patient care team members that can help build consensus for change and focus on plans for improvement; and 4) give teams a way to measure progress in this component of the Chronic Care Model. An online, interactive version of the PCRS is available at http://improveselfmanagement.org The online PCRS calculates team scores automatically and stores assessments so teams can easily monitor progress over time.

Also available are two worksheets. They can be used by both teams and management to guide, record and monitor improvement processes. The first is a PDSA-style worksheet adapted for use with the PCRS and the second is a companion report to capture the quality improvement process.
