Group Medical Visit Starter Kit (PDF)

This 11-page group medical visit Starter Kit plus appendix was developed by the New River Health Association, a partner of the Department of Family and Community Health at Marshall University. It describes a model that is loosely based on the Cooperative Health Care Clinic (CHCC) model developed by John Scott, MD and his colleagues at Kaiser Permanante-Colorado. The kit is designed for health care teams who want to begin offering group visits for their patients. It contains sample information to guide the process of getting started and references about how others have conducted medical group visits.
