Take Action*: Ready for Change Lesson Plan, Student Material and Food Diary and Behavior Worksheets (PDF)

The Take Action Diabetes Self Management Program material was developed by the Galveston County Health District in Texas City, TX. This section on readiness for change contains a lesson plan for health professionals to use while providing diabetes education. Accompanying student material for patients are included along with behavior and food diary worksheets.

The readiness for change materials are part of a comprehensive set which focuses on additional diabetes self management topic areas including blood sugar monitoring, complications, sick days, coping with diabetes, stress, exercise, medication and more. They can be found under various topic headings on this website.
