A
strength of the Diabetes Initiative is collaborative learning
to achieve quality improvements in diabetes self management
programs and services. Over the course of the Initiative, Collaborative
Learning Network Meetings and annual conferences offered opportunities
for skill building, networking and product development among
grantees, the NPO and experts in the field.
October
2006: RWJF Diabetes Initiative Capstone Meeting
The Diabetes Initiative hosted its Capstone Meeting in Tucson, AZ on October 18-20, 2006. Read Conference Proceedings from the meeting.
The meeting was a forum for launching dissemination of lessons learned from the Initiative. The 14 programs of the Initiative showcased their work to improve diabetes self management related to four themes:

The meeting kicked off with a keynote by Dr. James Gavin III who discussed trends in diabetes care and the central role of self management. Congresswoman Diana DeGette, U.S. Representative, Colorado, First District and co-chair of The Congressional Diabetes Caucus, discussed current policy initiatives related to diabetes. David Bornstein, author of How to Change the World: Social Entrepreneurs and the Power of New Ideas, gave an inspiring talk about his insights from social entrepreneurs. The meeting also included a talk on healthy coping in diabetes management and a panel to discuss self management from a payer's perspective. Several groups important in the Initiative's dissemination were invited and sent representatives to the meeting, including AADE, ADA, ASTHO, CDC, Diabetes Forecast, IHS, NGA, NIDDK, NDEP, OBSSR, Pfizer Health Solutions, private foundations, state health departments, other RWJF National Program Offices, our National Advisory Committee, RTI Evaluation staff, RWJF, and universities.

April 2006: Collaborative Learning Network
The Robert Wood Johnson Foundation Diabetes Initiative held
its sixth Collaborative Learning Network (CLN) meeting in
St. Louis, MO on April 4-6, 2006. The meeting kicked off with
with a session on sustainability and building the case for
self management support lead by Dr. Kerry Kilpatrick from
the University of North Carolina Chapel Hill. There was a
lively and interactive session on key messages lead by Ken
Fields from Fleishman-Hillard Inc. Grantees also had opportunities
to interact with a media panel to glean tips and strategies
on ways to work with media to share program findings. Media
panelists included Deborah Shelton from the St. Louis Post-Dispatch,
Robert Frederick from KWMU, St. Louis NPR affiliate, and Al
Wiman formerly with KSDK-TV and KMOV-TV and currently with
the St. Louis Science Center. Program director Ed Fisher presented
healthy coping protocols and grantees discussed workgroup
projects related to depression, partnership mapping, and community
health worker evaluation. In attendance were Diabetes Initiative
grantees, program staff, RTI Evaluation staff and invited
speakers.

October 2005: Third Annual RWJF Diabetes
Initiative Meeting
The Robert Wood Johnson Foundation Diabetes Initiative held
its 3rd Annual Meeting entitled "Self Management: Sailing
to Success" in Providence, RI on October 18-20, 2005. Invited
speaker Beverly Schwartz from Ashoka provided a lively presentation
on the topic of using strategic communication methods to disseminate
grantee program findings, Representative Sean Faircloth (Bangor,
ME) discussed the usefulness of such program findings to effect
policy, and National Advisory Committee (NAC) member Russell
Glasgow lead a discussion and group exercise on program sustainability.
Promotion of non-smoking as a central component of diabetes
management was the focus of a presentation by Ed Fisher, National
Program Office Director. Kelly Nevins from the Volunteer Center
of Rhode Island responded to grantee feedback for information
on supervising and working with volunteers. Ronald Aubert,
MedcoHealth Solutions and NAC chairman led an informative
discussion about Medicare. The meeting also provided perspectives
from the diabetes national scene: Christopher Laxton and Malinda
Peeples, American Association of Diabetes Educators;
Dara Murphy, Centers for Disease Control and Prevention; Ann
Albright, California Diabetes Program, University of California,
San Francisco; and Anne Weiss, Robert Wood Johnson Foundation.
In the spotlight, were grantees from Community Health Center,
Middletown, CT; Montana-Wyoming Tribal Leaders Council, Billings,
MT; and Open Door Health Center, Homestead, FL, who had the
opportunity to present their programs to the grantees. All
grantees also displayed and shared their work during a poster
viewing and networking session on the evening of October 18th.
In attendance were Diabetes Initiative Grantees, National
Program Office Staff, RWJF Program Staff, RTI International
External Evaluation staff, National Advisory Committee members,
and invited experts.

Network
The Robert Wood Johnson Foundation Diabetes Initiative held
it's fifth Collaborative Learning Network (CLN) Meeting in
St. Louis, MO on April 6 -7, 2005. In attendance were the
Diabetes Initiative Grantees, Program Staff, RTI International
Evaluation Staff, and invited speakers. This CLN focused on
a variety of topics including physical activity, dissemination/
sustainability, ongoing follow up and support, and emotional
health. Grantees from Richland County Health Department, Galveston
County Health District, and Department of Family and Community
Health/Marshall University School of Medicine gave In the
Spotlight presentations.

November 2004: Collaborative Learning
Network
The Robert Wood Johnson Foundation Diabetes Initiative held
its fourth Collaborative Learning Network (CLN) Meeting in
Las Vegas, NV on November 16 - 17, 2004. In attendance were
the Diabetes Initiative Grantees, Program Staff, members of
the National Advisory Committee, a member of RTI International
Evaluation Staff, and invited speakers. Dr. John Foreyt and
Dr. Debra Haire-Joshu discussed weight management in diabetes
focusing on lifestyle interventions and environmental and
policy support for weight management. Grantees from Campesinos
Sin Fronteras, Metro Denver Black Church Initiative, and Gateway
Community Health Center gave "In the Spotlight" presentations
on lessons learned and models used with their programs. Grantees
also prepared poster presentations on the topics of culture
or a significant program highlight from the first 6 months
of their implementation phase.

August 2004: Chronic Disease Self Management
Program Training
The National Program Office of The Robert Wood Johnson Foundation
Diabetes Initiative hosted a Chronic
Disease Self Management Program Master Training in Chicago,
IL on August 17 - 20, 2004. Teams from nine Diabetes Initiative
grantee sites and the National Program Office completed the
training to offer workshops and train other leaders in their
respective locations. There will be a follow up discussion
on implementation strategies at the fall Collaborative Learning
Network meeting in St. Louis.
July 2004: Second Annual RWJF Diabetes
Initiative Meeting
The Robert Wood Johnson Foundation Diabetes Initiative held
its second Annual Meeting, entitled Diabetes Self Management:
The Bridge to Better Health, in San Francisco, CA July 14
- 16, 2004. Keynote speakers Dr. Felix Aguilar and Amy Friedman
discussed clinic-community collaborations and Dr. Kate Lorig
lead a discussion and exercises on developing key messages and promoting self efficacy for self management. During a
Poster Viewing and Networking session on the evening of July
14th grantees had the opportunity to display and share their
work. Sharing continued throughout the meeting with a grantee
sessions called In the Spotlight. Holyoke Heath Center, Inc.,
MaineGeneral Health, Minneapolis American Indian Center, and
Providence St. Peter Family Practice Residency presented aspects
of their project. Other highlights included a socio-drama
presentation by promotoras from La Clinica de La Raza-Fruitvale
Health Project, Inc. and invited speaker presentations on
a variety of topics such as project sustainability. In attendance
at the Annual Meeting were the Diabetes Initiative Grantees
for both the Advancing Diabetes Self Management and the Building
Community Supports for Diabetes Care programs, National Program
Office Staff, the National Advisory Committee, RWJF Program
Staff, the Research Triangle Institute International External
Evaluation team, and invited experts.

March 2004: Collaborative Learning Network
The Robert Wood Johnson Foundation Diabetes Initiative held

Meeting were the Diabetes Initiative Grantees for both the
Advancing Diabetes Self Management and the Building Community
Supports for Diabetes Care programs, the National Program
Office Staff, a member of the National Advisory Committee,
a member of the RWJF Program Staff, and Invited Experts in
the field. The overall purpose of this meeting was to: 1)
Present Key Topics in Diabetes; 2) Allow for ample grantee
workgroup time; and 3) Discuss grantee improvement plans for
linking interventions with intended audience.

Network
The Robert Wood Johnson Foundation held its second Collaborative
Learning Network (CLN) Meeting in St. Louis, MO on December
10th and 11th, 2003. In attendance at the CLN Meeting were
the Diabetes Initiative Grantees for both the Advancing Diabetes
Self Management and the Building Community Supports for Diabetes
Care programs, the National Program Office Staff, a member
of the National Advisory Committee, a member of the RWJF Program
Staff, and Invited Experts in the field. The overall purpose
of this meeting was to: Discuss a plan for future CLN meetings;
Discuss the upcoming Phase II Call for Proposals; Provide
information on key skills in promoting and teaching self management;
Provide training on improvement cycles; and To allow for ample
grantee workgroup time.

July 2003: First Annual RWJF Diabetes
Initiative Meeting
The Robert Wood Johnson Foundation Diabetes Initiative held
its first Annual Meeting in Keystone, CO July 30 - August
1, 2003. A Building Community Supports for Diabetes Care program
specific meeting took place on July 29th, just prior to the
opening of the Annual Meeting. In attendance at the Annual
Meeting were the Diabetes Initiative Grantees for both the
Advancing Diabetes Self Management and the Building Community
Supports for Diabetes Care programs, the National Program
Office Staff, the National Advisory Committee, the RWJF Program
Staff, and Invited Experts. Titled Self Management: Keystone
to Quality Diabetes Care , the meeting objectives were to
provide an overall forum for collaboration and networking
among grantees, provide updates on best practices in diabetes
and diabetes self management, provide an overview of the Diabetes
Initiative evaluation plans, and discuss next steps and future
direction of the Diabetes Initiative Phase II.
